As a participant in Dominate Lacrosse events, I agree to the following:

1. Respect: I will show respect to all coaches, teammates, opponents, officials, and spectators.

2. Sportsmanship: I will play fair, follow the rules, and represent my team with integrity.

3. Effort & Attitude: I will give my best effort at all times and maintain a positive attitude.

4. Responsibility: I will be on time, prepared, and accountable for my actions both on and off the field.

5. Safety: I will play in a safe manner and report any injuries or concerns to a coach or staff member immediately.

I understand that failure to follow this Code of Conduct may result in disciplinary action, including removal from the event or program without refund.
